This positioninvolves partnering with physicians to support remote monitoring of ChronicHeart Failure (CHF) and Chronic Obstructive Pulmonary Disease (COPD), as wellas ensuring timely response to urgent clinical information such as critical labvalues. Nurse Practitioners will work one-hour to two-hour shifts daily reviewing urgentescalations of remote monitoring and lab data flows.
