This role is responsible for enrollingeligible patients, coordinating ongoing care for an assigned panel, andensuring continuity of care for individuals with chronic conditions. Theposition blends patient outreach, care coordination, and in-clinic support toenhance patient outcomes and engagement.</span></p><p class="MsoNormal" style="margin-bottom:0in"><b><span style="font-size:11.0pt;line-height:115%;font-family:"Calibri",sans-serif"><br></span></b></p><p class="MsoNormal" style="margin-bottom:0in"><b><span style="font-size:11.0pt;line-height:115%;font-family:"Calibri",sans-serif">Key Responsibilities:<o:p></o:p></span></b></p><p class="MsoNormal" style="margin-bottom:0in"><ul><li><span style="font-size:11.0pt;line-height:115%;font-family:"Calibri",sans-serif">Identify and enroll eligiblepatients into the Chronic Care Management program, including educating patientson program benefits and obtaining informed consent.</span></li><li><span style="font-size:11.0pt;line-height:115%;font-family:"Calibri",sans-serif">Manage an assigned panel ofCCM patients by conducting routine outreach and monthly follow-ups to promoteadherence to individualized care plans.</span></li><li><span style="font-size:11.0pt;line-height:115%;font-family:"Calibri",sans-serif">Coordinate patient careneeds in collaboration with providers, including referrals, medication refillrequests, lab orders, and diagnostic testing.</span></li><li><span style="font-size:11.0pt;line-height:115%;font-family:"Calibri",sans-serif">Facilitate appointmentscheduling, specialist referrals, and ongoing care coordination.</span></li><li><span style="font-size:11.0pt;line-height:115%;font-family:"Calibri",sans-serif">Accurately document allpatient interactions and care coordination activities within the electronicmedical record to ensure compliance with CCM program requirements and billingstandards.</span></li><li><span style="font-size:11.0pt;line-height:115%;font-family:"Calibri",sans-serif">Communicate patient updates,concerns, and care needs effectively to the clinical team and providers.</span></li><li><span style="font-size:11.0pt;line-height:115%;font-family:"Calibri",sans-serif">Provide patient educationrelated to chronic disease management and prescribed treatment plans.</span></li></ul></p><p class="MsoNormal" style="margin-bottom:0in"><b><span style="font-size:11.0pt;line-height:115%;font-family:"Calibri",sans-serif">Clinic Support:<o:p></o:p></span></b></p><p class="MsoNormal" style="margin-bottom:0in"><ul><li><span style="font-size:11.0pt;line-height:115%;font-family:"Calibri",sans-serif">Deliver in-clinic support toproviders as needed.</span></li><li><span style="font-size:11.0pt;line-height:115%;font-family:"Calibri",sans-serif">Travel between Amherst andNorthampton clinic locations based on provider schedules.</span></li></ul></p><p class="MsoNormal" style="margin-bottom:0in"><b><span style="font-size:11.0pt;line-height:115%;font-family:"Calibri",sans-serif">Schedule & Compensation:<o:p></o:p></span></b></p><p class="MsoNormal" style="margin-bottom:0in"><ul><li><span style="font-size:11.0pt;line-height:115%;font-family:"Calibri",sans-serif">Hourly Rate: $23–$25 perhour</span></li><li><span style="font-size:11.0pt;line-height:115%;font-family:"Calibri",sans-serif">Location: Amherst andNorthampton, MA (rotation between sites required)</span></li><li><span style="font-size:11.0pt;line-height:115%;font-family:"Calibri",sans-serif">Schedule: Monday–Friday, 40hours per week</span></li></ul></p><p class="MsoNormal" style="margin-bottom:0in"><b><span style="font-size:11.0pt;line-height:115%;font-family:"Calibri",sans-serif">Qualifications:<o:p></o:p></span></b></p><p class="MsoNormal" style="margin-bottom:0in"><ul><li><span style="font-size:11.0pt;line-height:115%;font-family:"Calibri",sans-serif">Certified Medical Assistantrequired</span></li><li><span style="font-size:11.0pt;line-height:115%;font-family:"Calibri",sans-serif">Minimum of 1 year ofexperience in a medical office or primary care setting</span></li><li><span style="font-size:11.0pt;line-height:115%;font-family:"Calibri",sans-serif">Prior experience with carecoordination, patient outreach, or Chronic Care Management preferred</span></li><li><span style="font-size:11.0pt;line-height:115%;font-family:"Calibri",sans-serif">Proficiency with EMR systems(Athena experience preferred)</span></li><li><span style="font-size:11.0pt;line-height:115%;font-family:"Calibri",sans-serif">Strong organizational,communication, and interpersonal skills</span></li><li><span style="font-size:11.0pt;line-height:115%;font-family:"Calibri",sans-serif">Ability to travel betweenAmherst and Northampton clinic locations as needed</span></li></ul></p> |
