This is a full-timetraveling position covering all boroughs, providing wound care services to PACEparticipants (elderly population) in the field.<o:p></o:p></p><p class="MsoNormal" style="margin-bottom:0in"><o:p> </o:p></p><p class="MsoNormal" style="margin-bottom:0in"><b>Compensation: </b>$75-80+ per hour, pending experience <o:p></o:p></p><p class="MsoNormal" style="margin-bottom:0in"><b><o:p> </o:p></b></p><p class="MsoNormal" style="margin-bottom:0in"><b>Responsibilities<o:p></o:p></b></p><p class="MsoNormal" style="margin-bottom:0in"><b><o:p> </o:p></b></p><ul style="margin-top:0in" type="disc"> <li class="MsoNormal" style="margin-bottom:0in;mso-list:l1 level1 lfo1; tab-stops:list .5in">Provide wound care assessment, treatment, and follow-up for PACE participants in the field.<o:p></o:p></li> <li class="MsoNormal" style="margin-bottom:0in;mso-list:l1 level1 lfo1; tab-stops:list .5in">Manage 4–5 patients per day, ensuring quality and continuity of care.<o:p></o:p></li> <li class="MsoNormal" style="margin-bottom:0in;mso-list:l1 level1 lfo1; tab-stops:list .5in">Collaborate with the interdisciplinary team to develop and implement individualized care plans.<o:p></o:p></li> <li class="MsoNormal" style="margin-bottom:0in;mso-list:l1 level1 lfo1; tab-stops:list .5in">Document care and progress in the EMR system accurately and timely.<o:p></o:p></li> <li class="MsoNormal" style="margin-bottom:0in;mso-list:l1 level1 lfo1; tab-stops:list .5in">Educate patients and caregivers on wound management and prevention strategies.<o:p></o:p></li> <li class="MsoNormal" style="margin-bottom:0in;mso-list:l1 level1 lfo1; tab-stops:list .5in">Maintain compliance with nursing standards, state regulations, and organizational protocols.<b><o:p> </o:p></b></li></ul><p class="MsoNormal" style="margin-bottom:0in"><b>Qualifications:<o:p></o:p></b></p><p class="MsoNormal" style="margin-bottom:0in"><b><br></b></p><ul style="margin-top:0in" type="disc"> <li class="MsoNormal" style="margin-bottom:0in;mso-list:l0 level1 lfo2; tab-stops:list .5in">Active Registered Nurse (RN) license in New York.<o:p></o:p></li> <li class="MsoNormal" style="margin-bottom:0in;mso-list:l0 level1 lfo2; tab-stops:list .5in">Wound Care Certification (required).<o:p></o:p></li> <li class="MsoNormal" style="margin-bottom:0in;mso-list:l0 level1 lfo2; tab-stops:list .5in">Current BLS certification.<o:p></o:p></li> <li class="MsoNormal" style="margin-bottom:0in;mso-list:l0 level1 lfo2; tab-stops:list .5in">Minimum 1–2 years of nursing experience; prior wound care experience strongly preferred.<o:p></o:p></li> <li class="MsoNormal" style="margin-bottom:0in;mso-list:l0 level1 lfo2; tab-stops:list .5in">Experience working with geriatric or PACE populations a plus.<o:p></o:p></li> <li class="MsoNormal" style="margin-bottom:0in;mso-list:l0 level1 lfo2; tab-stops:list .5in">Ability to travel to multiple boroughs; valid driver’s license preferred.<o:p></o:p></li></ul> |
