This role is partof a Program of All-Inclusive Care for the Elderly (PACE). tab-stops:list .5in">Advocate for participants and their families to ensure they receive comprehensive, compassionate care.<o:p></o:p></li> <li class="MsoNormal" style="margin-bottom:0in;mso-list:l0 level1 lfo2; tab-stops:list .5in">Assess and evaluate psychosocial functioning, including conducting mental status assessments.<o:p></o:p></li> <li class="MsoNormal" style="margin-bottom:0in;mso-list:l0 level1 lfo2; tab-stops:list .5in">Assist participants with navigating complex issues such as adjustment to illness/disability/treatment, financial concerns, and environmental challenges.<o:p></o:p></li> <li class="MsoNormal" style="margin-bottom:0in;mso-list:l0 level1 lfo2; tab-stops:list .5in">Provide crisis intervention, end-of-life care, and emotional support to participants and their families.<o:p></o:p></li> <li class="MsoNormal" style="margin-bottom:0in;mso-list:l0 level1 lfo2; tab-stops:list .5in">Collaborate with the Interdisciplinary Care Team (IDT) and community partners such as hospitals, skilled nursing facilities (SNFs), assisted living facilities (ALFs), and Adult Protective Services (APS).<o:p></o:p></li> <li class="MsoNormal" style="margin-bottom:0in;mso-list:l0 level1 lfo2; tab-stops:list .5in">Participate in discharge planning and care coordination to ensure smooth transitions between care settings.<o:p></o:p></li> <li class="MsoNormal" style="margin-bottom:0in;mso-list:l0 level1 lfo2; tab-stops:list .5in">Enlist and connect participants to community resources and agencies to address unmet needs.<o:p></o:p></li> <li class="MsoNormal" style="margin-bottom:0in;mso-list:l0 level1 lfo2; tab-stops:list .5in">Deliver education to participants and their caregivers to promote health, wellness, and independence.<o:p></o:p></li> <li class="MsoNormal" style="margin-bottom:0in;mso-list:l0 level1 lfo2; tab-stops:list .5in">Master of Social Work (MSW) from an accredited college or university <o:p></o:p></li> <li class="MsoNormal" style="margin-bottom:0in;mso-list:l2 level1 lfo3; tab-stops:list .5in">Active New York State LMSW or LCSW license</li><li class="MsoNormal" style="margin-bottom:0in;mso-list:l1 level1 lfo4; tab-stops:list .5in">Minimum of one (1) year experience in a healthcare setting – required.<o:p></o:p></li> <li class="MsoNormal" style="margin-bottom:0in;mso-list:l1 level1 lfo4; tab-stops:list .5in">One (1) year experience working with a frail or elderly population preferred.<o:p></o:p></li> <ul style="margin-top:0in" type="circle"> <li class="MsoNormal" style="margin-bottom:0in;mso-list:l1 level2 lfo4;
