<p class="MsoNormal" style="margin-bottom:0in">We are searching for a full-time BehavioralHealth Case Manager to serve as a member of a multi-disciplinary patient careteam for our Behavioral Health Division in Mesa, AZ. The Behavioral Health Case Managerprovides case management services that support the treatment goals of thepatients care plan. Medical,Vision, and Dental Insurance, Retirement Benefits, Paid Time Off, TuitionReimbursement and Educational Leave, Employee Recognition Program.<!--EndFragment--></o:p></p><p class="MsoNormal" style="margin-bottom:0in"><b>Qualifications:<o:p></o:p></b></p><ul><li style="margin-bottom: 0in;">Bachelor’s degree in social work is required.</li><li style="margin-bottom: 0in;">Master’s Degree in Social Work or relatedfield is preferred.</li><li style="margin-bottom: 0in;">Two years of related case managementexperience is required. Experience working with underserved and diversepopulations is preferred.</li><li style="margin-bottom: 0in;">Possess current and unrestricted Arizonadriver’s license with no DUI/DWI or reckless driving convictions in the lastfive years.</li><li style="margin-bottom: 0in;">Must present a current Class I FingerprintClearance Card prior to employment.</li><li style="margin-bottom: 0in;">Must pass alcohol/substance abuse testing uponemployment, and submit to random testing during the course of employment.</li><li style="margin-bottom: 0in;">Must have an updated Immunization (IZ) Record.</li><li style="margin-bottom: 0in;">Must have a current (within 12 months)Tuberculosis (TB) skin test or x-ray report upon employment and provide anupdate annually.</li><li style="margin-bottom: 0in;">Must obtain a CPR and First Aid Card withinninety (90) calendar days of employment and maintain valid updated trainingduring employment.</li><li style="margin-bottom: 0in;">COVID Vaccination Required</li></ul> |
