As a vital member of the Patient careteam, the focus is on recognizing the patient and family as the central unit ofcare, assisting with psychosocial adjustments for those dealing with chronic orterminal illnesses. Social Work cases are managed through formal and informalreferrals from various sources, and the LCSW-C will demonstrate expertise inunderstanding theoretical and bio-psychosocial factors, including the interplayof social, psychological, and health influences.
